This thesis focuses on depression, examining its clinical representations, treatments, and determinants through a sociological lens, while incorporating concepts from psychiatry. The first part analyses depression prevalence, utilizing data from various global and national sources to highlight its increased diagnosis and explore the term "epidemic." It critically evaluates whether this increase constitutes an actual epidemic, delving into the sociological explanations for the rise in depression, contrasting social change theories with diagnostic practice explanations. It argues that depression's depiction as a widespread issue is influenced by social, historical, and cultural contexts. The second part presents an empirical investigation into depression as a sociological object, using qualitative methods like semi-structured interviews, participant observation, and document analysis in two healthcare settings in Milan, Italy. The study explores both professional and patient perspectives, examining representations of depression among mental health professionals and the social dynamics involved in depressive and anxiety disorders in patients. It also addresses the diagnostic and treatment practices concerning depression, considering the biopsychosocial model. The thesis contributes to understanding the complexity of depression and its treatment in the context of public healthcare systems.
Questa tesi si concentra sulla depressione, esaminandone le rappresentazioni cliniche, i trattamenti e le determinanti attraverso una lente sociologica, incorporando anche concetti dalla psichiatria. La prima parte analizza la prevalenza della depressione, utilizzando dati da varie fonti globali e nazionali per evidenziare la sua diagnosi in aumento e esplorarne l' "epidemia". Valutando criticamente se questo aumento costituisca una vera epidemia, si approfondiscono le spiegazioni sociologiche per l'aumento della depressione, contrapponendo teorie di cambiamento sociale con spiegazioni relative alla diagnosi. Attraverso un'ottica costruttivista, la rappresentazione della depressione risulta un problema diffuso sia influenzata da contesti sociali, storici e culturali. La seconda parte presenta un'indagine empirica sulla depressione come oggetto sociologico, utilizzando metodi qualitativi come interviste semi-strutturate, osservazione partecipante e analisi di documenti in due contesti sanitari a Milano, Italia. Lo studio esplora sia le prospettive professionali che quelle dei pazienti, esaminando le rappresentazioni della depressione tra i professionisti della salute mentale e le dinamiche sociali coinvolte nei disturbi depressivi e d'ansia nei pazienti. Affronta anche le pratiche diagnostiche e di trattamento relative alla depressione, considerando il modello biopsicosociale. La tesi contribuisce a comprendere la complessità della depressione e del suo trattamento nel contesto dei sistemi sanitari pubblici.
