This report provides a clinical case of an adult male with extensive necrotizing fasciitis that required multiple organ support and was successfully managed through flap reconstruction. Despite the delivery of broad-spectrum antibiotic therapy, the patient’s clinical status worsened in less than 12 hours after ICU admission, with the onset of multiorgan failure (MOF). Emergency surgery enhanced organ function and weaned the patient off supportive organ therapies. Following surgery, hyperbaric oxygen treatment was considered. The delay in surgery appears to be a critical element in the treatment sequence of large necrotizing fasciitis.
