Anterior dislocation of the shoulder can be associated with rotator cuff tears and also with nerve palsy. Rockwood describes this association of injures as "the terrible triad of the shoulder", its incidence increase with age. The diagnosis of this injures is the first problem. The rotator cuff tear can be initially misdiagnosed, furthermore the nerve palsy adds important difficulties to the diagnosis. The treatment of this injures is the second problem. In literature there is a small number of cases of terrible triad, all the authors recommended early surgically repair of the rotator cuff tears, without explorating the nerve lesion. We introduce a case of terrible triad of the shoulder with also dislocation of long head of brachial biceps (LHB).
