Objective: to describe the use of spontaneous maternal postures during labour, in a low-risk population with a diagnosis of occiput posterior (OP) position at the beginning of the active labour. Methods: a retrospective observational study with 336 women. Results: 249 babies (85.6%) rotated in occiput anterior (OA) at birth. The majority of the foetuses rotated during the second stage of labour; the squatting position (60.6%) was the posture most commonly used, followed by the lateral one (53%). Discussion: women with a baby in OP should be encouraged to move spontaneously during labour as babies could be facilitated to rotate in OA.
