CAROTID STENTING AND CIRCLE OF WILLIS MALFORMATION - Carotid endarterectomy (CEA) has proved its worth in the prevention of stroke, with efficacy superior to medical therapy alone, in patients with symptomatic and asymptomatic carotid stenosis greater than 50-60%. In more recent randomized clinical trials, angioplasty and stenting of the carotid artery (CAS) demonstrated comparable efficacy to traditional surgery (CEA). We report a case where the ab- normality of the circle of Willis would not have allowed tolerance clamping. It was then practiced carotid stenting with full clinical success. The therapeutic choice between medical therapy, CEA or CAS will always be set according to the clinical features of the individual case.
STENTING CAROTIDEO E MALFORMAZIONE DEL CIRCOLO DI WILLIS - L’endoarterectomia carotidea (CEA) ha dimostrato la sua validità nella prevenzione dello stroke con efficacia superiore alla sola terapia medica nei pazienti sintomatici ed asintomatici con stenosi della carotide maggiore del 50- 60%. In studi clinici randomizzati più recenti, l’angioplastica e stenting della carotide (CAS) ha dimostrato un’efficacia paragonabile a quella dell’intervento chirurgico tradizionale (CEA). È descritto un caso clinico in cui l’anomalia completa del circolo di Willis non avrebbe consentito la tolleranza al clampaggio. È stato dunque praticato stenting carotideo con pieno successo clinico. La scelta terapeutica tra Terapia Medica, CEA o CAS andrà sempre posta valutando le caratteristiche cliniche del singolo caso.
